1. Diseases/ surgery/ history of dysphagia (Parkinson’s,
stroke, cancer)?
2. Pulmonary status (intubation?
Tracheostomy/COPD/pneumonia)?
3. Current nutritional intake (Nil By mouth,
NG-tube, PEG)
4. Medical status (SpO2 level <90,
respiration rate 40+?, activity level?)
· SpO2 Level (<90?)
· Respiration rate +40?
· Activity level (bed bound?)
· Weight loss/ dehydration/ malnutrition
5. Cognition/ mental status - ability to
